Middleware that adds a header value to the HTTP extensions

It seems like a server middleware that extracts a predefined header value from the incoming HTTP request and adds it to the extensions, so that operation handlers can use them, while easy to implement, is general and useful enough that it should probably be vended by smithy-rs.
