Quiz forms are missing for Pre-Lecture Quizzes

Describe the bug
"Form is not found" error or wrong link when I try to take the pre-lesson quiz

To Reproduce
Steps to reproduce the behavior:

1. Go to 'https://microsoft.github.io/Web-Dev-For-Beginners/#/'
2. Click on 'Lessons' on the sidebar
3. Click on any of the linked lessons on the lesson table
4. Click on the pre-lecture quiz link
5. See error - this goes to a page that says "This form doesn't exist
   Please make sure the link is correct and try again."

I was expecting it will link me to a form for the pre-lecture quiz to see my current abilities with web development

I'm not really sure if this is an issue but I also did clone the repository on my Github but same results otherwise.
