Sparse external arrays via index column

For some of the v2.0 proposals I have put forward sparse external arrays would be required to make them practical. However, since we are just dealing with one-dimensional arrays all we need is to add an index column to the external array format (the "internal" arrays already have indices so could stay as they are).
