Sometimes when CentOS6 or 7 is updated, and pakiti-client is run, a duplicate host is created in "Hosts" list. The report is not added to the existing host but a new one is created.

How to troubleshoot this issue?

I've noticed this too, but it seems to be caused by assuming that one host has exactly one IP-address, which is never the case in the dual stack world. It creates separate entries for IPv4 and IPv6, sadly.

Have not found a fix yet, except maybe removing legacy IPv4 address from the DNS.
